How do I?

Order a transcript?

Transcripts can be ordered online via the National Student Clearinghouse. Or you may submit a paper Transcript Order Form (in the forms list below) directly to the Registrar’s Office in Seattle by scanning and emailing it to transcripts@cityu.edu. Transcripts ordered through the online system at the National Student Clearinghouse are $13.00 USD per copy. Transcripts ordered via a paper order form are $18.00 USD per copy. Shipping may be extra.

Express ship my transcript?

CityU offers First-Class mail service by USPS and an express mail service by eShipGlobal called UEMS (“University Express Mail Service”), for shipping all documents and forms. First-Class Mail transcript orders are processed within 2-3 business days and are sent from the University Registrar’s Office via first-class U.S. mail. Secure Electronic PDF official transcripts are ONLY available for transcript requests through National Student Clearinghouse. Express Domestic Shipping for within the U.S. re processed within 2-3 business days and are sent from the University Registrar’s Office via UPS for an additional $20 USD per order. Express International Shipping for outside of the U.S.is available for an additional fee via eShipGlobal’s University Express Mail Services (UEMS). If using Express International Shipping, an official transcript order must still be placed through the National Student Clearinghouse, or via a paper transcript order form with all related fees paid. A second process for express shipping requires creating a shipping label through UEMS in which the student chooses options for shipping and pays this shipping fee in advance. Request a deferment?

Students may request a deferment by completing a Direct Loan Deferment Request form or an In-School Deferment Request Form (from the forms list below). Please send the completed form to the Registrar’s Office by scanning and emailing to Registrar@cityu.edu.

Request an enrollment verification?

If you are a U.S. student you have the option to obtain an Enrollment Verification through the National Student Clearinghouse. All students worldwide may request an enrollment verification letter by contacting the Registrar’s Office at Registrar@cityu.edu. Please include your full legal name, CityU Student ID number, program of study, and year of start.

Update my personal information on file with the University?

Most contact information (address, email, phone) may be updated online through the Student Center located on the Student Dashboard. Official name and directory information need to be updated using a Name Change Request Form or a Change in Directory Information Form (paper in the forms list below or online via the link below) as both require official signatures and/or additional documentation to update. Forms may be mailed to the University attention Registrar’s Office or scanned and emailed to Registar@cityu.edu. Online forms will submit directly.

Prevent the release of my directory information?

Please take a moment to watch the following presentation regarding student privacy. You may submit a Prevent Release of Directory Information Form ((paper in the forms list below or online via the link below) as both require official signatures and/or additional documentation to update. Forms may be mailed to the University attention Registrar’s Office or scanned and emailed to Registar@cityu.edu. Online forms will submit directly.

Release my education records to an approved 3rd party?

You may submit a Consent to Release Education Records Form (paper in the forms list below or online via the link below) as both require official signatures and/or additional documentation to update. Forms may be mailed to the University attention Registrar’s Office or scanned and emailed to Registar@cityu.edu. Online forms will submit directly.

Order a copy of my diploma or certificate?

You may request another copy of your diploma or certificate by submitting a Reproduction of Certificate/Diploma Application (paper form from the list below) to the Registrar’s Office by mail or by scanning and emailing the completed form to graduation@cityu.edu. A re-issue fee of $50.00 USD will be charged. If you required an additional apostille or authentication please visit All Things Graduation to find out how to request this additional service.
